<?xml version="1.0" encoding="UTF-8"?><Articles><Article><id>186</id><JournalTitle>MEDICATION USE SYSTEM CHALLENGES AND DEVELOPMENTS</JournalTitle><Abstract>	With disease variation, treatment individualization, and the rapid influx of pharmaceutical options, there is a growing room for error. This review will discuss medication safety aspects with a focus on pharmacistâ€™s responsibilities in the prevention of medication errors and maintenance of quality use of medicine. This review also aims to address possible faults with the current medication labeling system that lead to inefficiencies and its impact on patients. Also, medication reconciliation as a solution to overcome communication difficulties between practitioners in various settings. The review hopes to reiterate the necessary participation by the patients themselves in preventing medical errors.
